Palm Desert Street Fair  located at 43-500 Monterey Ave, Palm Desert CA  92260
with Limits of $1,000,000 per Occ/$2,000,000 Agg.      Policy term of 10/01/11 - 10/01/12.

Fantastic Indoor Swapmeet located at 1717 So. Decatur, Las Vegas, NV 89102
with Limits of $1,000,000 per Occ/$5,000,000 Agg.      Policy term of 2/01/12 - 2/01/13.

Orange County Marketplace located at 88 Fair Drive, Costa Mesa, CA 92626
with Limits of $1,000,000 per Occ/$2,000,000 Agg.      Policy term of 3/01/12 - 3/01/13.

Please note no coverage is included for the following activities:  medical test or exams; blood tests; body piercing; permanent/temporary tatoos; face painting; liquor sales; retail stores; motor sport activities; haunted house attractions; fireworks and weapons; inflatable devices (must be on stand alone policy); rope ladders; climbing devices of any type; games with shooting projectiles including cork guns, bean bags, etc; metal tip darts; non-caged animal acts/attractions; petting zoos; all mechanical rides including mechanical bulls, orbitrons; rental of licensed or unlicensed vehicles, scooters, wheelchairs, baby strollers wagons, shopping carts, demonstrations.

Policy excludes Patent, Trademark, Copyright Infringement & Medical Payments. Subject to Intellectual Property Exclusion. Please note that Additional Insured coverage applies as per insured’s operation under written contract only. This policy does not include Property, Auto, Workers Comp or other forms of coverage that should be obtained under a separate policy. This policy does not automatically renew. It must be applied for and quoted each year. The policy will be effective the day signed documents and payment is received. Policy will expire per the above dates regardless of  effective date. Fee shall be deemed fully earned when paid and is not subject to refund or return.                          License #0B99800
